Millions of women across India live with PCOS symptoms, yet many spend years without a clear diagnosis or answers. Now, a landmark global consensus has officially renamed the condition to PMOS, polyendocrine metabolic ovarian syndrome, bringing a far deeper understanding of how it affects the whole body. We sat down with Dr Laxmi Sangolli, OB-GYN, IVF specialist, and associate professor at Shree BM Patil Medical College, to break down the PCOS vs PMOS difference, rethink PMOS treatment, and understand what this PCOS name change means for every woman navigating this condition today.

Part 1: Why PCOS Has Been Renamed to PMOS?

Judith: PCOS, polycystic ovary syndrome, is now officially renamed as PMOS, polyendocrine metabolic ovarian syndrome. This is a landmark global consensus decision. What is your take on this, and what does it really mean for women?

Dr Laxmi Sangolli: PCOS is a well-known problem that exists in the majority of women all over the world. The thing is, only the name has been rectified, because the earlier name would not justify the condition. The major problem was that the disease does not exist only in the ovary. It exists in the overall body. It is named polyendocrine because multiple hormones are involved, and it is a metabolic syndrome, meaning it involves all organs of the body, not only the ovary. The ovary is just a part of it. This renaming is so that everybody understands. Men should not blame women, saying, you have PCOS, so you have not conceived. They should know it is polyendocrine, it is metabolic, and the ovary is only a part of a larger syndrome.

Dr Laxmi Sangolli: Let me explain the PCOS causes. In a regular menstrual cycle, FSH, that is, follicle-stimulating hormone, dominates the first half, and LH, luteinizing hormone, dominates the second half. In patients with PCOS, this ratio is altered. There is LH dominance. High LH causes increased androgen levels, meaning testosterone is high. That leads to androgenic features like hirsutism, which is excessive body hair, and acne. It also causes irregular cycles because ovulation does not occur as it does in a regular 28-day cycle.

Dr Laxmi Sangolli: PCOS presents with irregular cycles, acne, hirsutism, and obesity, plus polyendocrine features like insulin resistance. The renaming highlights that the main cause is not the ovary. The main cause is endocrine. PCOS is not only a condition for reproductive-age women. It affects adolescent girls and perimenopausal women too. It can cause cardiovascular diseases, diabetes, and thyroid problems. Patients feel fatigue, depression, and mood swings. They have shifted the name from ovary to endocrine and metabolic, and that is the essence of this renaming.

Judith: So you are saying the disease remains the same, but the renaming now includes the endocrine and metabolic dimensions, so it is not just confined to the ovaries. It is the overall body that is affected.Dr Laxmi Sangolli: Exactly. We have to evaluate the overall body.

Part 2: Can I Get Pregnant with PMOS?

Question from a Premom user: “Does PMOS mean I will have difficulty getting pregnant? I am 26 and planning to start trying.”

Dr Laxmi Sangolli: 26 is a very appropriate age to conceive. The best age for reproduction is 20 to 35 years. Ovarian reserve is very good at this age, and there is less chance of aneuploidy, meaning genetically abnormal babies or repeated abortions.

Dr Laxmi Sangolli: She has PMOS, but I will not say she does not have a good chance of conceiving. She has a good chance. She just needs a little help, and that help can start at home. Premom has given so many options, like ovulation prediction kits. Even with irregular cycles, she can use OPKs. The formula is simple. Take your cycle length and subtract 17 days. That gives you the day to start testing. Suppose her cycle is 45 days. Subtract 17, and she starts on day 28. She tests daily or twice a day until the test line matches the control line. That is the LH surge peak. She is ovulating, and those are her best days to conceive, her fertile window.

Dr Laxmi Sangolli: She can try this for 2 to 3 months on her own. If it is not happening, she can then visit a doctor for an ultrasound follicular scanning, where the dominant follicle reaching 16 to 18 mm is tracked, and an HCG trigger can be given at the right time.

Dr Laxmi Sangoll: PMOS fertility is not the same as infertility. She can conceive. She will need some guidance, but definitely she can. 

Judith: So you are saying that with PMOS, it is not leading to infertility. With the right knowledge, OPK testing, and support, getting pregnant is very much possible.

Dr Laxmi Sangolli: Absolutely.

Part 3: PMOS Without Obesity or Abnormal Hormones

Question from a Premom user: “I have PMOS, but I am not overweight. My doctor says my hormones are fine. Why do I still have symptoms?”

Dr Laxmi Sangolli: This is a very important question. Not all PMOS patients are obese. BMI (Body Mass Index) may be normal. Hormones may appear within the normal range, but they may be at the higher end of normal. And clinical experience matters here.”Sometimes the triggers are not obvious. Job stress, family pressure, or everyday anxiety can cause subtle hormonal shifts the patient may not even connect to her symptoms.” We call it subclinical. For example, subclinical hypothyroidism does not present obviously, but still causes mood swings and fatigue. These are all PCOS symptoms that can present without a textbook diagnosis.

Dr Laxmi Sangolli: In PMOS, even with normal BMI and seemingly normal hormones, there may be subtle hyperandrogenism, or LH levels on the higher side of normal. The patient thinks it is normal, but it is the higher side of normal.

Dr Laxmi Sangolli: Also, the term polycystic itself is a misnomer. They are not cysts. They are antral follicles, smaller follicles that produce more androgens. That is why hirsutism, or hair loss and weight gain, appears in many patients. The patient may present with only subtle symptoms, mild irregular cycles, mild hirsutism, mood swings, or just difficulty conceiving. Textbook-perfect presentations are not always there.

Part 4: PMOS Treatment and What You Can Do?

Dr Laxmi Sangolli: There is no single drug for PMOS. The first line of PMOS treatment is lifestyle modification. Walk. Aim for 10,000 steps. Reduce stress with yoga and meditation. Avoid junk food, and eat healthy vegetables and fruits. When you come to us as doctors, we evaluate your hormones. If the thyroid is low, we treat it. If insulin resistance is high, lifestyle modification helps that too. For obesity, medications like semaglutide or tirzepatide are now available alongside lifestyle changes. 

Dr Laxmi Sangolli: Symptomatically, irregular cycles have treatment. Hirsutism can be managed with laser therapy or medication. Acne has treatment through drugs, diet changes, and oil-free skincare.

Dr Laxmi Sangolli: One very important point. Do not allow your cycles to go beyond 90 days without seeking help. If you have not had a period in more than three months, please see a doctor. Prolonged anovulation significantly raises the risk of endometrial hyperplasia and uterine cancer. We can predict this, and we can prevent it, but only when addressed early.

Dr Laxmi Sangolli: PMOS patients are also at higher risk for diabetes, cardiovascular disease, and dyslipidemia. All of this can be managed when caught at an early stage. That is exactly why this condition needs to be understood in its full scope, not just as an ovarian problem.
